Roles and Responsibilities:

• Provide clinical services, including assessment, screening, brief therapy, and community referrals.
• Conduct biopsychosocial assessments and develop treatment plans for patients.
• Counsel patients to assess needs and readiness for treatment, while encouraging community living and independence.
• Develop and implement treatment plans, document changes in the Medical Record, and ensure ongoing patient progress.
• Select activities suited to individual capabilities and aid patients in developing a sense of responsibility.
• Collaborate with the treatment team, contribute to discharge planning, and provide ongoing documentation of services.
• Maintain regular contact with the supervisor and participate in professional development activities.
• Coordinate with community services for patient placement and assist in quality care.
• Build and maintain strong rapport with clients, offering both individual and group treatment.
• Comply with all security, confidentiality, and time attendance policies.

Roles and Responsibilities:

• Provide the full scope of primary care services, including diagnosis, treatment, coordination of care, preventive care, and health maintenance for patients aged 16 and above.
• Develop efficient, high-value care plans and communicate effectively with the care team.
• Conduct thorough patient evaluations, maintain accurate and timely documentation, and ensure medication reconciliation as needed.
• Order and interpret diagnostic studies, providing appropriate follow-up and specialist referrals as necessary.
• Collaborate with an interprofessional team to deliver high-quality, patient-centered care.
• Participate in quality improvement, patient care reviews, and continuing education activities.
• Maintain accurate documentation using AdvancedMD EMR.
• Remain calm and effective in high-pressure situations and provide direction to support staff.

Roles and Responsibilities:

• Adheres to Departmental procedures, fulfilling compliance with Life, Safety, Fire and Hazardous waste requirements.
• Triage/Coordinates and performs direct patient care and clinical flow.
• Prepares patients for providers, assisting with exams, assisting provider with OBAT navigation upon completion of OBAT training, carrying out orders, (i.e.; various supplies/tests/dispensing of medications.)
• Provides health assessments, preps for physicals, completes calibration of equipment, and performs various testing (i.e.; EKG, EEG, Xpert Xpress testing, drug testing, diagnostic testing).
• Enters and oversees data entry with ADVANCEDMD computer system.
• Stocks and orders medications and medical/surgical supplies.
• Reviews physical/tests results with provider and handles phone calls (i.e.; casemanagement, testing/physical results).

Roles and Responsibilities:

• Collect patient vitals and medical history
• Update medical records in the electronic medical records.
• Communicating with patients
• Arrange hospital admissions and laboratory services.
• Submitting patient tests to labs to be professionally reviewed.
• Assist during medical examinations.
• Handle receivable and payable accounts and keep financial records.
• Ensure that the medical facilities are sanitized before and after every patient.
• Administer cardio-pulmonary tests (EKG, EEG, stress test) on patients.
• Prepare and clean treatment rooms and medical instruments
• Additional clinical duties as needed.
