At Omega Physicians, we believe your care shouldn't pause between appointments. Our Remote Care Management program brings your doctor's support directly to your home using simple technology to monitor your health, keep your care team informed, and make sure nothing gets missed.
Whether you're living with a chronic condition or simply looking for more consistent support, this program is designed to keep you healthier, safer, and more connected to the care you deserve.
Programs We Offer:
Remote Patient Monitoring (RPM) – Track vital signs like blood pressure, blood sugar, and oxygen levels from home. Your care team reviews your data daily and reaches out if anything needs attention.
Remote Therapeutic Monitoring (RTM) – Stay on track with your treatment plan. Your team monitors your progress with medications, mood levels, therapy and adjusts your care as needed.
Chronic Care Management (CCM) – If you have two or more ongoing health conditions, CCM gives you a personalized care plan and a dedicated coordinator to help manage everything, from medications to specialist referrals.
Collaborative Care (CoCM) – Mental health support built into your primary care. If you're dealing with depression, anxiety, or substance use, our behavioral health team works alongside your doctor to support you.
Advanced Primary Care Management (APCM) – Whole-person care that goes beyond office visits, addressing your medical, behavioral, and social needs year-round.
How to enroll:
Getting started is simple.
Talk to your Omega physician: Ask about Remote Care Management at your next visit or by calling our office. Your doctor will recommend the right program or bundle based on your health conditions and goals.
We handle the setup: Our care team will verify your insurance, walk you through the program, and ship any devices you need directly to your home.
Start receiving care: Your dedicated coordinator will reach out to get you onboarded and answer any questions. Your care team will create personalized care plans for you and your Omega physician stays updated with your health data between visits. You get support and answers every day.
